ISM Prop 39 Re-enrollment form - URGENT info requested
Dear Parents,
We have sent you a previous e-mail regarding filling out a form re: your enrollment intentions for next year. I can't emphasize enough how important it is for you to return the form to our office right away. The purpose of the form is to justify our request for facilities for ISM for next year. We must have 100% replies and we have a due date to submit all the forms to MPUSD on Sept. 30. That is why we have asked you to return the form by Friday, the 26th of September.
The form can also be faxed to us at (831) 899-7653.
Please help and don't delay. For ISM to have a proper school facility for next year, we need your response. If you have already returned the form, thank you. Please note we do need the actual form filled out and returned, an email message indicating your child is returning is not enough.
